Lauren Ebels: We moved my father to skilled nursing here after having a poor experience elsewhere. They seem very attentive and intentional about involving him in activities. They take COVID precautions seriously. The facility is nice and the staff are welcoming. I only wish they had more options for freedom as he is almost completely wheelchair bound but still has the mental capacity and energy beyond most in skilled nursing and would prefer to be in assisted living.

Britney Stevens: Clark itself it a good place, it's just the food contracting company Unidine/Compass group (Unidine was bought by compass group) that suck. Sub par cooks for the Unidine staff minus the Clark oringal staff, they are great but not the people Unidine hired or brought in. So glad I quit. Best decision I ever made. I hear from Clark staff and residents still that say that Unidine don't order enough food still and sub out which affects the nutrition of these seniors. Hope Clark gets smart and gets rid of the food contractor and goes with a company that actual cares and knows what their doing.

Kat Foster: I can only speak for the parts of the facilities that I've seen. It has never looks dirty or overly aged and the staff has always been helpful and kind. I visit my grandmother there and she has been in independent living for a couple years now and has also been in rehab twice. Independent Living is much better than Rehabilitation. Rehabilitation of course never has enough staff ( true almost anywhere you go) and is not the most rigorous facility for quick recovery. But it is in-house so my grandmother was able to still be with her friends, which was more important. Since they change food vendor's, the quality has gone down and I have noticed when dining with her that the poor serving staff are forced to constantly apologize for this. ( something they have no control over) this is a very expensive facility and Extras will cost you extra money. But they have multiple stages of care which is what my grandmother wanted so she wouldn't have to get used to a new environment if she needed to change levels of care. It also has a Buy in program so if your money Runs Out you won't be kicked out period I think this brings a lot of Peace of Mind to my grandmother.
SKat Foster: Not perfect but they come close. As someone who was a caseworker for adults who couldn't live independently, and visited many, many care settings, I am aware that any facility that has as many residents and as many staff as Clark will have a few issues occasionally. It's the overall day to day care that matters.That said, we are very happy with Clark. My dad, who spent 2 days a week (for many years) visiting people in almost every care setting in the GR area, chose Clark for himself and mom. I think He made a good choice. He has passed on, but mom has been there several years. It's clean, in good repair, has many activities, and staff are kind and competent.We visit frequently at all hours, eat in their dining hall with mom at least once a week, and some of us have spent the night with mom. Mom is mentally all there and is also well satisfied. I'd recommend you consider it.

Elizabeth Parker: My parents moved there in the fall of 2005. Mom needed more care. She died in 2007. Dad lived at Clark until 2010 when he passed away. Both received very good care. I was at Clark at all times of day. The staff is caring, the buildings are clean, the food is good. I would recommend Clark. My family found great comfort with the policies of Life Time Care. If a Clark Resident outlives their money they can continue to live at Clark.

Alexis Vilardi: This place is awesome!! My dad loves it here, and I don't know what we would do without them. Dad is very happy with the food and loves the variety they offer. They also offer tons of opportunities for activities. He has been involved in Bible studies, exercise classes, and art projects, but there are several other options in addition to that!! They are always having fun holiday parties and celebrations as shown in the images of my dad I have shared. This past year, my dad was in the hospital over his birthday. One of the super awesome staff at Lincoln Square reached out to me to ask if they could send him well wishes somehow. The very next morning they had collected more than 40 birthday cards with notes and well wishes from all the residents and staff for me to bring to Dad in the hospital on his birthday. They made him feel so incredibly loved, and he has made some amazing friendships at Lincoln Square!!! The staff and other residents have become an extension of our family.

Cindy Stebbins: Several excellent reviews from family of residents! Food is very tasty according to several assisted living residents, all their plates were empty. All were clean and hair combed.
Beth Anne Gruizinga: I wish I could give a negative 10 as a score!! If you love your parent(s), I beg you NOT place them at Clark on Keller Lake!!! The beautiful building/setting increases the corporate chances that you will agree to the $6000/month fee and is a diversion to the reality of the situation!! There is no “functional” kitchen so they transport the food from another facility and it’s AWFUL!! Very little real meat (mostly processed), no fresh fruit or vegetables. I sat in on many meetings regarding the the poor quality of food, no accommodations for those who need a specialized diet, and the residents concerns. Lots of promises, but no follow through. Activities are insulting to those who have active minds and reminded me of early elementary school. My mom withdrew to her room due to the lack of stimulation. There are some wonderful staff, but then there are the others. My mom was hospitalized on many occasions due to neglect. Some involved not providing medication and others more severe. One of these experiences was when she got sick in the middle of the night in her bed. They put her in her recliner to change her bed and LEFT her there all night!!! She was violently ill and was covered in vomit and diarrhea by the time it was discovered in the morning. My mom was septic by the time she was taken to the hospital!! 2 weeks in ICU, myoclonic seizures, hallucinations and the doctors said to call the family for our final goodbyes. There was no apology from Nancy, the manager, or Tina,?the head nurse! If it wasn’t for the kindness of one of the workers who shared the truth, we would never have known! My sister and I demanded a meeting with Nancy who denied any wrongdoing and offered no sympathy whatsoever until I asked for the records for that night. Apparently, there were none. After pressing her, she finally admitted that she fired the staff on duty, but did not report them to LARAmi.gov so they were free to find jobs at other facilities! I NEVER got my final hug and kiss due to their inappropriate isolation.Don’t LET this happen to you and your parent(s)My goal is make the public aware and am slowly moving toward developing a case against Clark.
Laura: The ladies that take care of the residents are wonderful, very kind and compassionate. The way Clark has handled Covid has been terrible. Visitation for residents has opened up for the first time in a year, allowing residents to see their family members. Clark's policy is each resident can have one visit a week for one hour. Two people can visit at a time. One. Visit. A. Week. Wow. So siblings draw straws? Prisoners have better visitation policies than Clark. And the residents at Clark pay a fortune to live there. Pre-Covid, the meals were terrible. I don't know if they have improved but it was a whole lot of dry tuna sandwiches with lima beans for dinner. My family member had asked for oatmeal for months for breakfast due to heart issues and received bacon and eggs instead. I really like the grounds, the location and the staff but their policies are not meant to keep their residents happy, they are meant to save a buck. If Clark really cared about their residents they would put money and effort into make visits more readily available for family. This is what the residents really need right now. Family. Love. Someone to talk to. At Clark, they won't get it.

Laura Stapert-Meijia: Great community, great people, great food, amazing apartments, and the staff love their jobs and it truly shows! If you are looking for a forever home cascade trails is an amazing place.
Lori B.: Smoke and mirrors. If you’re looking for a new facility with activities, this is your place. If you want quality of care, forget it. We pulled mom after a couple of weeks due to lack of care and follow through on the “care plan”. The wellness director was not responsive, staffing was short handed and not supervised. Hint: One of the 5 star reviews here was written by the executive director! (Update: he identified himself and rewrote his review after this was posted which demonstrates the deceit)
Nikki VandeVoorde: Beautiful place but very unhappy with our uncle's end of life care. Great place until his final days. A United States Marine Corp Vietnam Vet should not spend his last days lying in dried urine. We complained only to have them not change his sheets but cover it with a bed pad. I guess when they know their monthly payment of over 7,000.00 a month is about to end so does their standards in care. If your loved one is in memory care well good luck getting in and out the locked doors. We had to wait for the kitchen staff since none of the nurses had a key fob but somehow a family member from another patient had a key fob and was able to let us out. Just know their marketing is fabulous. Maybe that's where all the money goes. I pray your relatives never have to spend their last days the way our uncle did.

S M: From day one this facility has struggled to care for my family member. On day one they allowed my family member to leave the facility which is supposed to be a locked door facility. From there it has only gotten worse. They allow my family member to sit in filth. Because she “denies” a shower the facility is ok with it and allows her to go days and almost weeks without giving her a shower unless family comes and assists. Physical therapy has denied her service because she refuses in the moment. She has Alzheimer’s and has no way of knowing what she is saying and yet they allow her to make these decisions on her own. It appears to simply be an excuse to not have to work with her. Her bathroom and bedsheets are often covered in fecal matter that has been there for days. Her bedsheets constantly are misplaced and we are told we need to buy more. She wears the same clothing for days and weeks at a time unless family comes and assists. When you voice the concerns to the staff and admins they won’t accept responsibility and make excuses. They make notes that they make a full assessment, however she has plates of moldy food and rashes on her body that go unattended. It is heartbreaking to know what my loved one faces in this facility and god only knows how many other families have loved ones in the same facility that are going through the same thing. No one seems to really care unfortunately.

I was communicating with a person or supervisor.at that facility on Burton Street Southeast community,a couple of times concerning the waiting list. But the Samaritans on 32nd Street does not have the communication skills to help seniors like me to find where they need to go for help.today I've tried calling Samaritanas, customer service, line three to four times each time I I was asking how may I direct your call well the phone number that I have for my last conversation with someone on the Burton Street Southeast community is the number that I called but instead it gave me the 30 second Street as I tried to explain to the customer service my situation they were very rude. not willing to help. put me on hold and left me there. after waiting for 5 to 10 minutes. I call back no one answered. the telephone operator said I had the wrong number. It doesn't take a genius,to figure out that they didn't want to answer the phone. after my third conversation.with the customer service, totally unprofessional. I hope nothing like this happens to one of their family members. I'm a senior in need of a place to live independently. Is the kind of help that you get. My question is where are the adults. That are getting paid to help people like me find the right directions, connections.Thanks!
Sharon Shelley: I have been impressed by the entire staff at Samaritas. There are always nurses and aids around even though they are under staffed. They are caring and kind to the patient as well as visitors. The Physical Therapy and Occupational Therapy are exceptional. Food is not always to his taste but I know it is hard to please so many.
Sarah Morren: Samaritas started off well, but is falling apart. The laundry room can't bother to keep grandma's undergarments from mysteriously disappearing multiple times. To allow an elderly person to sit all day without underwear when we have purchased and repurchased underwear for her is absolutely ridiculous! I can't help but find the lack of care disrespectful. Since the pandemic, her room has deteriorated too. Items hanging on the walls have fallen or are hanging on by a thread producing a disheveled room. It is hard to standby and watch a care facility that you entrusted your beloved family member with allow such preventable occurrences.
Debbie Rolf: My mom has been at Samaritas The Lodge now for over 2 years with Dementia.They are an awesome group of caretakers!! Her care has been excellent there.I have seen a little turnover but in fact my mom has had the same Aids and nursing staff now for over the 2 years.They are caring wonderful people who take care of each resident as if it were their own loved one!Recently, my mom contracted a mild case of Covid and had to be sent to a Covid Hub. It was a horrible place and experience SKLD on the Beltline in Grand Rapids.After the lack of care I witnessed there I am counting our Blessings that we have Samaritas!They are a Christian Based organization and it shows in the loving giving care they provide every day to help the elderly! Thank you Samaritas for all you do!
Kayla Benefiel: Absolutely horrible place. Multiple times, we have entered my grandmother's room to find dirt all over her floor and feces all over her toilet seat. On the back of the toilet was a dish rag with fecal matter on it. When we found my grandma, she was in the dining area, practically falling out of her wheelchair. A nurse was sitting right next to her, but was ignoring her and was instead focusing on eating. When we asked the nurse to help us move her back to her wheelchair, she refused and said that she wasn't my grandma's caretaker that day. An absolute disrespect and lack of care for the residents and horrific hygienic practices and filthy living conditions. Do not send your loved ones here!
Kasha Cockrell: I have it in good authority this place has a high turnover rate and is never adequately staffed. 1 aide is responsible for 32 residents due to staff calling in. The aide doesn't get a lunch no break this is against labor laws. Leadership may want to investigate before the state comes in.

Shannon vanderploeg: My dad has been living at the lodge since December. It was quite an adjustment for him at first. But now he has made a lot of friends there. The facility is very nice. The nurses are wonderful! But you do need more help on 3rd shift and on the weekends because the amount of patients the facility has. You also need a new call system with lights outside the door so the staff know when patients need help. Relying on pagers doesn't always work.
Brenda Dehoek: Residents have to stand in line 30 min and wait for meals when there are 25 empty tables because there is not enough dining room staff to serve meals. Residents are not seated in an orderly manner; whoever fusses the most gets seated first. Visitors have to pour coffee and water for the residents because the few staff members are so overwhelmed. This has been going on for months.
Tanya Gomez: I worked at the Woods for almost two years. It was terrible. They have a high turnover rate for employees so they hire anyone. The things that they let the staff get away with was just wrong. Residents would go hours without care until you would have to ask someone else to help. I can not count on two hands how many times I caught employees sleeping in residents rooms. The head nurse was amazing but any other management was useless. They have no idea what happens on the floor so when you go to them for help they act like they care but nothing is done. When working with residents you have to care for them like family but so many employees just look at it as a job and slack. Everyone calls in on the weekend so you have to scramble and with 3 people get over 50 residents up and dressed. The scheduler doesn't like to help with your schedule because it is not convenient for her and will mess up her master schedule. There are some very good workers at the Woods that have been there for many years but other than them no one seems to care. They put residents into this facility that should not be there because they are completely paralyzed and then they don't provide the equipment to move them safely. I feel so bad for anyone that puts their family into this facility. I deeply miss the residents I worked with daily.

The staff obviously don’t enjoy their job, it smells like someone’s smeared poop throughout the building and I’m mortified my grandmother has to live here.She was left in the bathroom for a ridiculous amount of time and when I asked the aide if it was legal she shrugged her shoulders, said yes, and made excuses of not having enough staff......If you don’t have enough staff you shouldn’t be accepting new people. This is unfair, disrespectful, and a poor choice of response.Definitely not a place I would suggest keeping your loved ones at.

Sentinel Pointe Retirement Community: what do users think?
Aura Heinz: I have nothing but good things to say about this place. Sherry took the time to answer all our questions and she listened to everything my mom and I had to say. The food at the restaurant was great (A+ egg salad!) with really attentive (but not pushy) wait staff (high five to Brandon). The front desk people were very nice (I even watched them talk to a couple residents and they were nice to them too). The apartment is spacious. Everything makes me glad my mom chose this place to move into now that she can't do steps easily.
Renee Simmons: My parents moved in a couple of years ago. This was a difficult transition for them. The staff at Sentinel Pointe helped make it easier. They are the tremendous and truly caring individuals who step in where needed to make SP a comfortable place for so many. In addition, the size of the rooms, the beautiful views of trees and wildlife ( deer, turkey, owls, black squirrels and many birds) make for a wonderful attraction.

Janet Chambers: Sentinel pointe offers a good value for a smaller senior living facility. During COVID lockdowns, the staff kept the residents staff during a difficult time and communicated with family members about what was happening. The staff know the residents by name and are very friendly. While there have been challenges with staffing, just like all businesses, I think the staff has rallied and tried to fill in the gaps.

When she was unsure, others there stepped in to help her. The daily interaction for meals and fun activities have brought out a whole new confident person! She loves it here! And the meals are exceptional! We are grateful for finding a place that is engaging, caring, safe, and with a staff that is second to none.
Laurie Potter: Upon our first tour of Sentinel Pointe we were greeted by friendly, caring staff in a beautifully appointed and clean facility. Charming apartments, meals, housekeeping and many social activities are part of the Sentinel Pointe experience. The apartments allow independence and privacy and the staff provide just the right amount of support if/when needed. The activities are endless and provide wonderful opportunities to make friends and socialize at any comfort level. Sentinel Pointe Retirement Community is a great option for those looking for a stress free environment in their golden years.
Willie Perkins: My experience differs from my fathers experience. I am very pleased because I see a staff who really cares. There are a lot of activities. It always looks very clean. However, there is a serious problem keeping some of the staff.
Susan Clark Denny: I loved Sentinel Pointe when my Mom moved in 5 years ago! Classy dining room, lovely apartment, fine quality through and through. I especially appreciated the caliber of the food offered at meals - not just Gordon Food Service crud. Alas, I got Covid while I was visiting Mom at Christmas, and witnessed first-hand the deplorable food you were now giving residents. No balanced nutrition. Actually, no nutrients AT ALL! Nothing freshly prepared, almost no vegetables, nothing that wasn't coated in a layer of breading. I was served one of the Gordon's frozen chicken pieces that literally had more coating than meat! I don't know how a diabetic survives there anymore. And it certainly isn't worth what my Mom is paying for her food service. And the one meal I had in the dining room - there were no tablecloths or cloth napkins. NO CLASS. NO QUALITY.
Sandy Vanven: I am at Sentinel Pointe weekly to visit my brother and the people are all so friendly, every time. The staff seem to take pride in their work and it shows. If I leave a message for a staff person, it is generally returned pretty quickly. I would gladly recommend SP to anyone. So far, so good. Thank you for a job well done.

XxxCoraXxx z: This place used to be one of the best around but very poor management and Covid has really taken this place down. They used to have staff that loved to work there and it showed. Now, they hardly have staff. Food is terrible and no hope in sight for the dining services. Note that most good reviews are over a year old. With all the retirement facilities around I would strongly urge you and your family to look elsewhere.
Stephen Dertz: We moved my mother to Sentinel Point in 2018. The facility became her home as much as she resisted settling into her new apartment. The staff and the other residents are open to new people in the community and made her feel welcome and involved. Mom passed away in January and with the loving help of Faith Hospice, the Sentinel nursing staff and everyone at Sentinel Point, her passing was as peaceful as we could have hoped for.It’s a wonderful place, filled with caring and compassionate people. You can’t go wrong having a loved one join the community when it’s necessary.

I have said that if you get hungry, thirsty or bored at Sentinel Pointe, you work at it! To me, living at Sentinel Pointe is like being on a cruise ship that is always tied up at the dock. Welcome aboard!A big plus for me: Flexible dining hours. I can have breakfast from 8:00 AM – 2:00 PM. Lunch is any time between 11:00 AM and 2:00 PM. At lunch and dinner, we have our choice of more than ten menu items and four seating times for dinner. As another resident says, ‘I don’t think about what to have, I don’t shop for it, I don’t cook it and I don’t clean up.”I have a nice apartment, three very good meals every day, my apartment is cleaned once a week and they launder my bed linens. I don’t fold sheets anymore! I enjoy the activities and have made great friends. We also have medical staff on duty around the clock. That’s a comfort to my children.It’s impossible to overstate the excellence of the staff here. They exhibit a characteristic that makes all the difference in the world. They care “about” us as well as well as caring “for” us. It’s a subtle and powerful difference.I’m happy here!

Mark Lawrence: The facility is very nice and the lunch served to my friend looked good. The staff were all very friendly and helpful. My complaint is that atmosphere when I was there seemed really boring for the people living there. What kinds of activities are there for the residents and how often do they occur? A variety of activities throughout the day would be very helpful. Maybe you do that and I just didn't see it because I was there at lunch time.

Mark Pierson: This place is a train wreck, at least in the memory care facility. I know it is difficult to find a good memory care facility for a family member and I sympathize with those who are looking. I pray you find a good location for your loved one and that is why I am leaving this review. Staff is in turnstile mode, hard to keep consistency in care when there isn't consistency in staff. The biggest issue is with management, my father spent two extra days in the hospital as a social worker tried to get them to respond to see if he could return. When we spoke with the manager and indicated he was moving as they could not provide him the care required, the manager indicated to us she hadn't made that evaluation and was the only one who could do it. We also haven't gotten returned phone calls on items that went missing from his room when he went into the hospital. We also pulled up one day to the facility (this was spring of this year) and a resident who was supposed to be inside, was outside of the secure area. There is much more to the story and there were some very caring providers during my dads stay, but our overall experience wasn't positive.

Most of the aides had terrible attitudes, were mean, and they lost all her laundry even though they were labeled with her name. They seem not to care at all about the patients. There were 4 very kind aides, but the rest were not fit to work in a place with aging people.

The Village of Heather Hills: what do users think?
Jessica Carey: Facility is not maintained. Their abandoned building they own behind the facility has several broken windows and trespassers that management has been unwilling to deal with for months. The grounds have large dead falling trees that have broken neighboring fences and childrens play structures making our backyards a hazard for our kids. As a neighbor to the facility I’m so disappointed by the lack of pride they take in maintaining their property and the lack of care for the local community.
Linda Blanchard: After an exhaustive search for an assisted living facility for my mom in law I came to find Charlie Martinez and Heather Hills. What a blessing! Charlie immediately made me feel welcome and the love she shares with you, the staff, and every resident at the facility was above and beyond anything I have ever experienced! There was one resident in hospice care (she gave no details) but the pain on her face as she expressed her feelings for this resident made me know this is where my family member needs to be. This place is just different. The residents have choices, the place is beautiful and spotless, and everyone is just the best. And my mom in law just loves it there! She feels independent while having people with her if she needs anything. It is just wonderful. I simply cannot say enough about the quality of the facility and especially their staff. They make the difference. And Charlie is someone special. Thank you Heather Hills for making this transition for our family a positive experience!
Charles Howe: Heather Hills staff and caretakers are caring and sensitive to patient needs and dignity. As a Hospice caregiver, I have never visited a more caring and giving community. I especially want to recognize Gina, who leads the exercise classes and pays special attention to every resident. She makes everyone feel like they are her favorite. Additionally, Margaret, who oversees the veterans program, has a heart of gold . She goes to extraordinary lengths to research each Veteran's history and recognize their individual contributions to our democracy.The dining staff makes dining together for residents feel like a nice restaurant outing. The personal care staff is respectful of patient privacy and very responsive.I highly recommend Heather Hills for your loved ones.
Shelly Sichta: I cannot say enough good things about Heather Hills and its staff. I had just six days to find a place for my mom to move in the Grand Rapids area, following an auto accident that left her unable to live alone. After visiting five assisted living communities, I felt Heather Hills was her best choice. The facility is beautiful, and there are so many wonderful activities offered to its residents. More importantly, the staff is incredible, on every level – so kind and caring. Unfortunately, mom passed three weeks after moving in due to complications from the auto accident, but I was comforted knowing she was safe and in good hands while there, and for that, I will be forever grateful.

I have been able to partake in happy hour corn hole games. The calendar is jammed packed with relevant activities to keep residents engaged --not just occupied. The newly renovated space is bright and inviting. Don't pass up an opportunity to visit this premier community.
KERRI ORDERS: Heather Hills is a wonderful home for my mother. All of the staff, management, receptionists, and nursing aids have been exceptional over the past few years. They are caring, flexible and friendly. It feels like a second home to me when I visit my mom!
